Provides professional nursing services to accomplish the individual patient plan of care; independently and consistently using the nursing process to safely, therapeutically, and efficiently care for a full assignment of patients with subtle and sometimes unidentified needs. Support the operations of the Chugachmiut Health Services programs.
The functions of the RN Case Manager are to independently and consistently use nursing knowledge, collect patient health data, analyze the data to determine diagnoses, identify expected outcomes individualized to the patient, develop a plan of care that prescribes interventions to attain the expected outcomes, implement the interventions identified in the plan of care, and evaluate the patient’s progress toward attainment of the outcomes, working collaboratively with the healthcare team. Focus should be placed on preventative care services as well as necessary follow up on various individuals with acute and chronic conditions across the region.
- Alaska license in good standing as a Registered Nurse
- Current BLS required
- Experience in primary care services preferred
- Experience in rural health care setting preferred
- Experience in tribal health setting preferred
Hours worked are 8:30 a.m. to 5:00 p.m. with an hour for lunch. This a full-time 37.5 hours per week and 1950 hours a year position. Chugachmiut maintains a drug and alcohol free, non-smoking environment. There may be occasional travel to the villages of the region, must be willing and able to travel in light aircraft.
